1) Vessel safely moored against dolphins in the Caland kanaal. When the inspector approached the vessel by means of tender no contact could be made using tender whistle and using required VHF channels in the Rotterdam port area no adequate radio watch was held according to local rules- rectified-07 Jan. 2019 2) Found that the ship's is carrying a cargo not listed in IBC code chater 17 but in MEPC2/circ24 (provisional categorization). Key personnel was not aware of this.- Rectified 07 Jan. 2019. 3) Found only evidence that lifeboat has been lowered into the water in the last three months. Found no evidence that lifeboat also has been maneuvered in the water in the last three months- Rectified 07 Jan. 2019. 4) Found that the fourth Engineer is not familiar with the location of the local release of the fire fighting system(foam) mounted above the main engine. master instructed to hold a safety meeting with all engine room staff.-Rectified 07 Jan. 2019. 5) Found that the UV sterilizer light of the sewage treatment plant is not functioning.-Rectified 07 Jan. 2019. 6) Founds the drain of the leak tray mounted under the fuel oil filter of generator number 1 cloigged. A separate leak tray ( a tin) was installed to collect excess fuel oil.- Rectified-07Jan. 2019. 7) When opening the hot water taps in the hospital,the spare crew cabin and the sink in the galley brown colored water appeared. Opening and running of these taps is a monthly job inj the shiups management system. According to the records it was last done on 22 December 2018. Upon request the crew stated that only the cold water taps have been flushed. Rectified-07 Jan. 2019. 8) Corrective action taken on the ISM system by the Company is required within 3 months. Deficiency(s) marked ISM is (are) objective evidence of a failure, or lack of effectiveness of the implementation of the ISM code. The ship will be eligible for reinspection after 3 months from the final date of the report.- Rectified 12 March 2019
